In order to better serve your needs, we need to gather some information so we can begin the process of ensuring you have an stress-free wedding day. As our role in this is to coordinate the plan you have made, please make any edits or changes to this to suit your wishes for your wedding. Any sections not applicable to your wedding/reception please put n/a. If you are unsure or need our help with that section please leave blank.

**Additional Charges may occur for planning services or services outside of normal coordination tasks. We will discuss before we do any additional services that may incur additional charges.**

Once completed, returned and reviewed we will set up a meeting to go over details and ask any clarifying questions.
